La Odontopediatría es una especialidad de la Odontología que ofrece atención preventiva y terapéutica integral a bebés y niños hasta la adolescencia, incluidos aquellos niños con necesidades médicas especiales. Por ello, las técnicas y los procedimientos se adaptan exclusivamente a los requisitos de estos pacientes a diferencia de otras especialidades que se enfocan hacia el paciente adulto.
Al ser una especialidad, la odontología pediátrica abarca disciplinas como:
- El manejo de la conducta
- La educación preventiva de la caries dental
- La atención del paciente con problemas de desarrollo
- Supervisión del crecimiento y desarrollo dental – maxilar
- Tratamiento de la caries infantil
- Sedación
- Tratamiento farmacológico específico para la edad y odontología hospitalaria
Se recomienda la primera revisión dental a los 6 meses de edad para ayudar a los padres y familiares a establecer unos buenos hábitos de salud bucal que abarcan desde la higiene de encías y dientes hasta pautas para una correcta alimentación que evite la caries de primera infancia. Así mismo, recomendamos que la asistencia a la consulta odontológica debe realizarse como un acto cotidiano, rutinario, con refuerzo positivo de buena conducta para que desde pequeños se vayan familiarizando con el entorno y con el Odontopediatra, para que finalmente el hecho de acudir a la consulta sea una experiencia positiva sin miedos ni ansiedad.
El nuevo concepto de Odontología pediátrica se basa en prevenir la caries, para ello los consejos y asesoramiento tempranos ayudarán a los padres a tener todas las herramientas para lograrlo, además de unas adecuadas revisiones periódicas que nos ayudarán a controlar el desarrollo oral y a la aplicación de tratamientos odontopediátricos tempranos preventivos como la aplicación de flúor que deben de ir adaptándose a las necesidades individuales y edad de cada niño; así como los selladores de fisuras que están indicados en aquellos pacientes de alto riesgo de caries.
Si no se realiza un continuo seguimiento del paciente infantil pueden aparecer caries de primera infancia que presentan un patrón más agresivo y destructivo. Si no se tratan pueden provocar dolor, infección y pérdida de la función masticatoria de los dientes. Estos resultados pueden afectar negativamente al aprendizaje, la comunicación, la nutrición y otras actividades necesarias para un buen crecimiento y desarrollo.
Cuando la caries afecta al tejido pulpar del diente temporal o “ nervio del diente de leche” hay que realizar tratamientos conocidos como pulpotomía o pulpectomía; con ellos queremos mantener el diente afectado hasta su exfoliación natural.
Desafortunadamente, a veces llegamos demasiado tarde y hay que proceder a la extracción el diente afectado y la inmediata colocación de un mantenedor de espacio, ya que debemos preservar dicho espacio para la correcta erupción del diente permanente/definitivo.
Para evitar este tipo de patologías es obligatorio comenzar la higiene de los dientes con la erupción del primer diente de temporal o “de leche” que suele darse sobre los 6 meses de edad bajo los consejos y pautas de un Odontopediatra.
Otro campo importante del día a día de la consulta Odontopediátrica son los traumatismos orofaciales tanto en dentición temporal como permanente. Deben considerarse como una situación de urgencia y se recomienda acudir a la consulta en menos de 1 hora tras producirse el traumatismo. Es importante que el profesional que atienda esta situación de extrema urgencia posea amplios conocimientos Odontopediátricos y Endodónticos, ya que su manejo es muy diferente en la dentición temporal o “de leche” y en la dentición permanente o “definitiva”. Nunca debemos de restar importancia ante un traumatismo en un “diente de leche” por el simple hecho de que no es el “diente definitivo”, ya que en la mayoría de las ocasiones dichos traumatismos afectan a la formación de diente permanente, por lo que requieren de un control, seguimiento y tratamiento por parte de un Odontopediatra.
Gracias a un adecuado seguimiento del paciente infantil, podemos detectar anomalías en el correcto desarrollo dental y óseo que nos permiten realizar tratamientos de Ortodoncia interceptiva y Ortopedia que posteriormente facilitarán un tratamiento de Ortodoncia fija en el paciente adolescente. En determinadas ocasiones existe la falsa creencia de que la Ortodoncia empieza o debe evaluarse cuando el niño presenta todos los “dientes definitivos en boca” y no debería ser así. La mayoría de las alteraciones esqueléticas como la clase III (mandíbula muy pronunciada), clase II ( maxilar superior muy pronunciado), mordidas cruzadas, malposiciones dentarias localizadas, compresión del maxilar… se deben de tratar en el paciente en crecimiento para un favorable pronóstico en la edad adulta.
Por último pero no por ello menos importante, no debemos de olvidarnos de los pacientes con necesidades especiales de atención médica que incluyen aquellos con impedimentos físicos, de desarrollo, mental, sensorial, conductual, cognitivo o emocional, o cualquier condición que requiera un manejo médico especializado.
Consideramos que dichos pacientes entre los que englobamos a los niños con TDAH, enfermedades raras, síndrome de down… precisan de revisiones y tratamientos bucodentales para el cuidado de su salud oral. Desafortunadamente un alto porcentaje tienen grandes dificultades a la hora de recibir dicha atención odontológica por falta de colaboración o por la dificultad para efectuar tratamientos seguros en un medio no hospitalario ya que gran parte de estos pacientes requieren un tratamiento odontológico especializado bajo anestesia general en un hospital.

adminmoneo2020-06-02 10:08:352022-07-14 10:56:06EL PACIENTE INFANTILMantener una buena higiene en general es fundamental para evitar contagios. El cuidado de nuestra boca es igualmente importante a la hora de evitar la llamada “contaminación cruzada”.
- Jamás debemos “compartir” el cepillo de dientes: ¡es una manera de transmisión de enfermedades – cada uno el suyo!
- Cambiar el cepillo regularmente: normalmente el desgaste de las cerdas nos indicará cuándo hacerlo, sino cada 3 meses aproximadamente. En caso de contagio: desechar el cepillo inmediatamente después – para evitar la propagación en casa.
- Antes de comenzar el cepillado: lavar bien las manos con agua y jabón, pues los virus pueden pasar de la mano a la mucosa oral.
- Realizar el cepillado habitual (en caso de duda: ver vídeo de nuestra higienista “técnicas de cepillado”). Hacer uso del hilo/cinta dental, irrigador o cepillos interdentales para eliminar los restos acumulados en zonas donde no alcanza el cepillo.
- Una vez finalizado: enjuagar bien el cepillo debajo del grifo para eliminar restos de pasta, alimento, etc.… para extremar aún más las precauciones, podemos sumergir el cepillo y cabezal en una solución desinfectante. El Consejo General de Dentistas de España, así como diferentes publicaciones, proponen varias soluciones: [1] povidona iodada al 0.2% o [2] agua oxigenada diluida al 1% o [3] una solución a base de hipoclorito de sodio (lejía) durante 30 minutos, una vez pasado este tiempo, aclarar con agua y dejar secar.
- Secar bien el cabezal del cepillo, girándolo de izquierda a derecha para que suelte el resto de agua.
- Guardar el cepillo de forma individual y siempre boca arriba – no con otros cepillos de la casa – y alejados del inodoro (como sabéis el virus también está presente en las heces).
- Al finalizar: volver a lavarnos las manos con agua y jabón.

Causada a menudo por experiencias o traumas infantiles, temor al dolor, o experiencias de personas ajenas, es una de las principales causas del descuido bucodental. El paciente deja de acudir a sus revisiones, deja de realizarse los tratamientos necesarios para mantener la boca sana y por ello su boca empeora y comienza un círculo vicioso. Sin embargo, existe una técnica que permite dejar atrás la angustia y el temor de acudir al dentista: se trata de la SEDACIÓN CONSCIENTE, un procedimiento seguro e innovador, muy extendido en las clínicas dentales de Estados Unidos y el Reino Unido desde hace mucho tiempo, y que elimina toda sensación negativa a la hora de someterse a un tratamiento dental.
La principal clave de esta técnica es que la persona está plenamente consciente y con todas sus funciones vitales inalteradas, por lo que puede seguir las indicaciones del dentista mientras le realiza el tratamiento, pero sin sentir miedo o ansiedad. Además, dada la sensación de relajación que experimenta, existe la percepción que la duración del procedimiento es mucho menor a la real, algo que facilita mucho el trabajo al profesional. En este procedimiento se combina el control del dolor a través de la anestesia local en la zona a tratar y el control de la ansiedad con la sedación.
¿Qué es?
La sedación consciente es una técnica con aplicación intravenosa que tiene como objetivo obtener un estado de relajación y tranquilidad por parte del paciente, lo que contribuye a que se realice el tratamiento sin molestias y dolor.
¿Ventajas?
- Eliminación del temor al dentista y de la ansiedad.
- Mayor seguridad y estabilidad del paciente.
- Posibilidad de realizar todos los tratamientos en una sola sesión: implantes, tallados, endodoncias, extracciones…
- Completa colaboración del paciente y facilitación de la labor del odontólogo.
¿Para quién está indicado?
- Pacientes con estrés, miedo y ansiedad cuando tienen que acudir al dentista y someterse a un tratamiento.
- Niños: Las principales ventajas de emplear esta técnica en niños son la eliminación del miedo y la ansiedad y la percepción de dolor durante el tratamiento. Además, al no vivir su paso por la clínica dental como una experiencia negativa, previene el desarrollo de la fobia dental.
- Personas con discapacidad: La mayoría de las personas discapacitadas tienen dificultad para cooperar durante una consulta dental. Muchos pacientes con autismo, síndrome de Down o parálisis cerebral pueden ser tratados sin pasar por el quirófano. Con esta técnica disminuye el estrés, la ansiedad, el riesgo de aparición de crisis epilépticas y se atenúan movimientos involuntarios, haciendo posible realizar el tratamiento dental.

- Masticar:
Nuestro primer consejo es simple: coma alimentos “masticables”, tales como manzanas, zanahorias o nueces. El hecho de “triturar alimentos” hace que la placa dental se adhiera menos al esmalte dentario y a su vez ayude con una mejor digestión.
- El cepillo de dientes sí que importa:
Le recomendamos que elija un cepillo blando, pues debemos “barrer” la suciedad, no desgastarnos la encía y el esmalte.
- Elegir bien la pasta de dientes:
Para dientes sanos y fuertes, asegúrese de que su dentífrico contenga flúor. Si sufre de sensibilidad dentaria, gingivitis u otras patologías asociadas, opte por un dentífrico específico para dicha dolencia (su dentista le aconsejará).
- Limpie entre los dientes:
La placa dental se acumula fácilmente entre las piezas dentales y surgen las caries más peligrosas: las interdentales – que hasta no romper o doler no conseguimos apreciar en boca. Por lo tanto, debe usar hilo o cinta dental regularmente. También puede optar por cepillos interdentales o el uso de irrigadores (consulte con su especialista).
- El tiempo sí que importa:
Es importante cepillarnos los dientes después de cada comida o ingesta.
El ácido de los alimentos y bebidas genera placa, sarro y caries, por lo que no debemos permitir residuos durante mucho tiempo: lo más fácil ¡terminar de comer y cepillarnos! Si tiene dudas, consulte con nuestra higienista.
- Coma queso duro para tener los dientes sanos:
El calcio liberado del queso duro impulsa la desacidificación de la cavidad bucal y los dientes se vuelven lisos nuevamente.
- Té blanco para unos dientes bonitos:
Según varios estudios, se dice que el té blanco inhibe el crecimiento de las bacterias en boca: sobra decir que sin azúcares añadidos.
- Coma pescado para tener los dientes sanos:
Los pescados grasos omega-3 como el salmón o el arenque, contienen muchos factores saludables para nuestra boca. Tienen efecto antiinflamatorio y protegen las encías.
- El bicarbonato:
Si usamos bicarbonato disuelto en agua tibia para enjuagarnos bien la boca, neutralizamos la acidez. También es un buen remedio contra el mal aliento con el uso regular.
- Visite regularmente al dentista:
Una buena higiene, alimentación e hidratación bucal son fundamentales para mantener una boca sana y limpia, pero debemos acudir semestralmente al especialista para revisarnos y prevenir cualquier patología bucal que pueda surgir.
