La avulsión dental es la salida completa del diente fuera de su alveolo, es decir, la salida completa del diente tanto corona y raíz del hueso maxilar o mandibular.
¿Por qué ocurre una avulsión dental? La mayoría de las ocasiones son fruto de un fuerte traumatismo en la zona orofacial que hace que de desprenda por completo la totalidad del diente. Normalmente, suele ser dientes anteriores por lo que afecta tanto a la funcionalidad como existe un gran compromiso estético.
Por esta razón las probabilidades aumentan en aquellos pacientes que pueden sufrir golpes en la boca como aquellos que practican deportes de contacto, niños con edad entre los 7-10 años por actividades deportivas o simplemente por caídas o accidentes de tráfico.
¿Qué debemos de hacer si sufrimos nosotros mismo o alguien de nuestro entorno una avulsión dental?
- En primer lugar, debemos de mantener la calma y actuar con mucha rapidez
- Recogeremos el diente por la corona clínica del diente, nunca por la raíz.
- Lavaremos el diente con agua aproximadamente 10 segundos.
- Intentaremos recolocarlo en su lugar y acudir de forma urgente a una clínica dental de confianza.
- Si no es posible recolocar el diente en su alveolo, lo guardaremos en leche, suero salino o debajo de la lengua con mucho cuidado de no tragarlo y acudir rápidamente a nuestra clínica dental de confianza.
¿Debemos de hacer lo mismo si es un diente temporal o de leche?
La respuesta es NO. Los dientes de leche no se reposicionan puesto que al volverlos a colocar en su alveolo podemos dañar el germen del diente permanente. En estas ocasiones debemos de acudir a nuestra Odontopediatra de confianza y se valorará y evaluará el paciente, dando un plan de tratamiento correcto para este tipo de casos.
¿Qué hacemos cuando acude a la consulta un paciente con una avulsión dental?
Es de vital importancia que el paciente acude dentro de los primeros 60 minutos tras haber sufrido el traumatismo. Pasado ese tiempo, las probabilidades de éxito al reimplantar ese diente disminuyen considerablemente.
Si podemos reimplantar el diente en su lugar, ferulizaremos la pieza a los dientes de alrededor para conseguir que el diente carezca de movilidad durante 3 semanas y se pautará la medicación que sea necesaria en cada caso.
Una vez transcurridas esas tres semanas, se procederá a realizar la endodoncia de la pieza avulsionada.
Durante esas tres semanas es de vital importancia que se mantenga una dieta blanda, una higiene dental exquisita y evitar morder con el diente avulsionado.
¿Podemos evitar una avulsión dental?
La respuesta depende de cómo se produzca el traumatismo. Hay ocasiones en las que por ejemplo si ocurren por un accidente de tráfico o porque un paciente infantil se cae jugando en el recreo las probabilidades de evitarlo son mínimas. Por el contrario, si podemos evitar aquellas situaciones en las que la avulsión dental se produce a consecuencia de un traumatismo durante un deporte o actividad física habitual con el uso de protectores bucales.

A veces, posponemos mucho tiempo el hecho de reponer la pieza dental desde que la hemos perdido por desconocimiento de que ventajas presenta el tratamiento de implantes dentales o las consecuencias que pueden ocurrir si no lo ponemos en marcha.
Lo que la mayoría de las veces no sabemos, es que, cuanto más tiempo pase sin reponer esa pieza dental perdida pueden ocurrir cosas que jueguen en contra de nuestra salud bucodental. Quizás la más importante sea, dificultad para comer.
Al perder una pieza dental, contamos con una superficie dental menos para realizar una correcta masticación, lo que conlleva que ese trabajo ahora lo tengan que ejercer el resto de las piezas dentales, por lo que a mayor número de pérdidas de piezas dentales mayor serán los daños que puedan ocurrir en el resto de las piezas presentes. Entre ellos tenemos:
- Pérdida de hueso maxilar. Al perder la raíz del diente el hueso empieza a reabsorberse.
- Fracturas de piezas dentales al soportar la carga oclusal de las piezas perdidas
- Desplazamiento del resto de los dientes a consecuencia de los espacios vacíos.
Por tanto, en ocasiones, al demorar reponer esa ausencia, nos encontramos que el tratamiento que en principio era algo simple, ahora es más largo, complejo y costoso.
Es por ello, que siempre aconsejamos reponer la pieza dental lo antes posible y ¿Qué ventajas presentan los implantes dentales frente a otro tipo de rehabilitaciones protésicas?
- A día de hoy, la tasa de éxito del tratamiento ronda el 98% y en la mayoría de las ocasiones el paciente puede realizar su vida completamente normal al día siguiente.
- Es un tratamiento a largo plazo, ya que es una opción de tratamiento fija que funciona exactamente igual que un diente.
- NO DAÑAN ESTRUCTURAS DENTARIAS VECINAS. A diferencia de cuando realizamos por ejemplo un puente dental donde tenemos que tratar de forma irreversible los dientes cercanos a la usencia, con los implantes dentales el resto de las piezas se mantienen intactas.
- BUENA HIGIENE. El cepillado y el mantenimiento de los implantes dentales es igual al de un diente natural.
- Las coronas sobre implantes no se suelen diferenciar del resto de los dientes ya que se realizan con la misma forma y color que los dientes adyacentes.
- RENTABLES PARA EL PACIENTE. Al ser una opción de tratamiento con gran durabilidad a largo plazo aunque haya que realizar un esfuerzo económico inicial apenas requieren mantenimiento salvo sus revisiones periódicas.

Consiste en un movimiento involuntario que provoca el apretamiento o rechinamiento de los dientes pudiendo hacerse normalmente en horario nocturno, aunque también existe el bruxismo diurno siendo este menos frecuente.
Este habito conlleva a una serie de problemas como son:
- Desgaste dentario provocando sensibilidad dental.
- Inflamación de encías provocando gingivitis.
- Dolor mandibular
- Dolor en cabeza u oídos.
- Problemas en músculos del cuello.
En función del caso el bruxismo puede ser tratado por varias maneras:
- Usando férulas de descargas: Son férulas individualizadas para cada paciente y ajustada perfectamente a su arcada. Esta férula será revisada periódicamente para valorar el estado de la oclusión del paciente.
- Tratamiento de ortodoncia creando nueva oclusión.
- Infiltraciones de bótox: aunque el efecto es temporal se usa para el bruxismo severo ya que el efecto es inmediato.
- Fisioterapia: siempre debería combinarse con la férula de descarga.
Por lo tanto, lo más conveniente sería acudir a consulta para individualizar cada caso y así poder determinar el grado de bruxismo del paciente dando con el tratamiento adecuado.

Ambos tratamientos se encuentran englobados en la especialidad de la periodoncia, pero la gingivectomía es una intervención quirúrgica que busca reparar, mantener y sostener los tejidos de soporte dental, mientras que la gingivoplastia es un proceso periodontal quirúrgico que se realiza para remodelar las encías con el objetivo de mejorar el contorno gingival y obtener mayor estética dental y gingival.
GINGIVECTOMÍA
Con este tratamiento eliminamos el tejido gingival con la finalidad de eliminar o reducir las bolsas periodontales bajo anestesia local.
Las bolsas periodontales se forman cuando hay una pérdida importante del hueso que sirve de soporte al diente, creándose un espacio desde comienza la encía hasta el hueso maxilar o mandibular. En dicha bolsa suele producirse una acumulación de bacterias causante de la enfermedad periodontal.
Con este tratamiento, intentamos eliminar dicha bolsa periodontal o reducir su profundidad para que el paciente pueda tener una mejor higiene dental y frenar el avance de la enfermedad periodontal.
GINGIVECTOPLASTIA
Este tratamiento es muy parecido a la gingivectomía pero su finalidad es completamente diferente. Con ello lo que intentamos es devolver el contorno de la encía cuando éste no se encuentra correctamente, por lo tanto, tiene un objetico claramente estético siendo el resultado una sonrisa más bonita mejorando el tamaño de las encías, que a veces se ven demasiado al sonreír como es el caso de la sonrisa gingival.
Hemos de destacar que antes de realizar cualquiera de los dos tratamientos descritos anteriormente es necesario la valoración de cada paciente de forma individual y por parte de un periodoncista que podrá dar un diagnostico y tratamiento adecuado a cada caso.

La zona más afectada es la lengua.
Aparte de los signos mencionados, el paciente puede notar alteración del gusto y sequedad.
Suele afectar más a mujeres en estado de menopausia o postmenopausia, aunque también pueden verse afectados los hombres, la edad de aparición en ambos sexos es de 50-60 años.
En cuanto al ardor suele aumentar progresivamente durante el día, alcanzando su máximo pico al final de la tarde, puede llegar incluso a impedir el sueño, pueden presentar también cambios de humor, ansiedad o depresión.
FACTORES LOCALES:
Los factores que pueden desarrollar este síndrome el alcohol, que se puede encontrar también en distintos colutorios, factores biológicos, como hongos o bacterias, incluso pasta de dientes o cosméticos.
FACTORES SISTÉMICOS:
Entre ellos puede desencadenar el síndrome, déficit de vitaminas, como la B12,B6, ácido fólico y vitamina C.
Paciente con problemas de anemia, alteraciones hormonales e inmunológicas.
FÁRMACOS:
Fármacos que pueden producir el ardor bucal al producir menos cantidad de saliva, son los antihistamínicos, hipertensivos, antiarrítmicos, benzodiacepinas y neurolépticos.
TRATAMIENTO:
No hay tratamiento específico para el síndrome, pero si para que el paciente note mejoría.
- Se debe eliminar los factores locales o sistémicos que afecte a la mucosa, como alergias de contacto, controlar hábitos parafuncionales, y se deben instaurar terapias sustitutivas, en caso de déficit de vitaminas y minerales.
- Beber mucha agua ayuda a generar saliva, y así no desarrollar el síndrome.
- Evitar ácidos, bebidas carbonatadas y café, así como dejar de fumar.
- Usar pasta dentales sin sabor o suaves.

adminmoneo2023-10-03 13:22:032023-10-03 13:28:03Síndrome de boca ardienteEn el post de este mes queremos hablaros sobre un tema importante de salud actualmente, el cáncer oral, ya que unas 1500 personas fallecen al año debido a esta patología, más que por accidentes de tráfico.
El dentista, es clave en la detección precoz del cáncer oral, es por ello, que siempre recomendamos revisiones cada 6 meses o anuales para poder detectar cualquier lesión sintomática o asintomática que puedan aparecer en la cavidad oral.
Es, por tanto, que, si se diagnostica de forma precoz, la tasa de supervivencia es mayor al 90%.
Cuando un paciente acude a su revisión rutinaria, el odontólogo realiza una exploración tanto de las piezas dentales como de los tejidos blandos que incluyen mejillas, labios, lengua, paladar y encías.
Si en la visita detectamos cualquier lesión como manchas rojas y/o blancas, ulceraciones o bultos recomendaremos pruebas adicionales o derivación a cirugía maxilofacial para obtener más información sobre esa lesión.
No obstante, no debemos de olvidar que existen factores de riesgo que pueden aumentar la prevalencia de esta enfermedad:
- Mala higiene dental: la acumulación de placa bacteriana puede aumentar el riesgo de cáncer oral, es por ello, que insistimos en cepillar la cavidad oral, eso incluye dientes y tejidos blandos al menos dos veces al día, así como, utilizar seda o hilo dental.
- Alimentación poco saludable: una dieta de bajo consumo en frutas y verduras, así como con déficit en vitaminas y/o minerales aumentan el riesgo de padecer dicha enfermedad.
- Consumo de alcohol y tabaco: son los principales factores de riesgo del cáncer oral, constituyen el 90 & de las causas directas de la enfermedad, sobre todo si actúan sinérgicamente.
- Exposición al sol: la exposición prolongada a los rayos UV sin fotoprotector pueden aumentar el riesgo de padecer cáncer oral en la zona de los labios, por tanto es aconsejable utilizar protectores solares labiales cuando estemos expuestos al sol, como por ejemplo ahora en verano en las zonas centrales del día.
En las primeras etapas del cáncer oral puede pasar totalmente desapercibido, por lo que tenemos que prestar mucha atención a cualquier lesión anómala en la cavidad oral que no cura tras unas dos semanas.

Su déficit crea unos espacios llamados troneras, o vulgarmente “Triángulos negros”, los cuales suponen un compromiso tanto estético como funcional.
La etiología de los triángulos negros es multifactorial:
- Distancia entre los dientes.
- Distancia desde el hueso hasta el punto de contacto entre los dientes.
- Presencia de enfermedad periodontal.
- Biotipo gingival.
- Divergencia de las raíces. Tratamiento ortodóntico.
- Edad.
- Morfología dentaria.
Existen en la actualidad numerosas técnicas para la reconformación de las papilas, como:
Surge un tratamiento alternativo, mínimamente invasivo: el relleno de la papila interdental con Ácido Hialurónico.
Es un polisacárido del tipo glucosaminoglicano, presente principalmente en el tejido conectivo y líquido sinovial.
Entre sus propiedades:
- Hidrófilo.
- Biocompatible.
- Bacteriostático.
- Antiinflamatorio.
- Antioxidante.
El ácido hialurónico en el campo de la Odontología viene utilizándose, entre otras indicaciones en el área periodontal, como tratamiento no invasivo de los triángulos negros.
El protocolo de aplicación para el relleno de papilas consiste en la infiltración en la zona, en base al tamaño del defecto a tratar. Esta técnica ha demostrado un agrandamiento tisular en casos de pérdida de papila, solventando no sólo el problema de estética dental, sino también restaurando la barrera biológica y devolviendo a la papila sus funciones mecánicas.

Por ello, en ocasiones nos olvidamos de la rutina de nuestra higiene dental y olvidamos su correcto cuidado. Con esta entrada de blog queremos daros una serie de consejos para que podáis cuidar de vuestra sonrisa, pero a la vez disfrutar de unas merecidas vacaciones. ¡Vamos a ello!
- Intentar mantener la misma rutina que durante el resto del año. Siempre recomendamos al menos cepillar los dientes dos veces al día. Aunque si lo podemos hacer tres, ¡perfecto! Para no tener excusas, puedes adquirir un kit de limpieza dental de viaje que incluya un cepillo dental pequeño, una pasta de dientes e hilo dental. Con ello, podrás llevarlo en el bolso, mochila, cesta de la playa… y no descuidar tu higiene bucodental.
- Intenta mantener una alimentación saludable que incluya frutas y verduras variadas. Estos alimentos, junto con el pescado, la carne, frutos secos y agua nos ayudan a aportar los nutrientes necesarios sin un alto contenido en azúcar, el gran enemigo de nuestros dientes.
- Mantén una hidratación adecuada y evita las bebidas carbonatadas o cítricas, ya que estas últimas pueden llegar a ser dañinas para nuestros dientes ya que aumentan los niveles de ácido en el medio oral dañando al esmalte dental, provocando sensibilidad y mayor propensión a la caries dental.
- Modera el consumo de alcohol y tabaco, ya que la sinergia de estos dos hábitos puede aumentar la aparición de manchas en los dientes, alitosis (mal olor) y lo más importante, aumentan las probabilidades de aparición de cáncer oral.
- Con el sol, las altas temperaturas y la sal de las playas, nuestros labios pueden sufrir de sequedad. Aconsejamos hidratar tus labios con cacaos específicos que contienen protección solar para evitar quemaduras solares y vaselina para hidratarlos después de un buen día en la playa y/o piscina.
- Hay que tener cuidado con las aguas cloradas de las piscinas, puesto que poseen productos químicos que pueden llegar a erosionar el esmalte dental, así como la aparición de manchas color café.
- Por último, pero quizás lo más importante, hay que tener mucho cuidado con los traumatismos. Como realizamos más actividades deportivas y de ocio al aire libre tenemos mayor riesgo de sufrir una fractura o avulsión dental. Tener mucho cuidado y si se produjese acudir lo más pronto posible al Odontólogo.

Todo lo que comemos pasa por nuestra cavidad oral, por lo tanto vamos a darle a la salud bucodental la importancia que se merece. Para ello vamos a hacer referencia a los alimentos que no deberían faltar en tu dieta y los que deberíamos de prescindir para que tus dientes y encías sean saludables.
Alimentos necesarios en tu dieta:
- Leche y derivados: Proporcionan CALCIO, fundamental para la reparación y remineralización del esmalte dental.
- Frutas y verduras: las frutas nos proporcionan vitaminas y fibra. La fibra hace que produzcamos mayor cantidad de saliva y ésta conlleva a una eliminación de la placa bacteriana. Hay frutas como los arándanos que nos proporcionan gran capacidad antiinflamatoria a nivel de la encía, por lo que son estupendos para prevenir la enfermedad periodontal.
- Frutos secos: tienen alto contenido en minerales como el magnesio y calcio, fundamental para la estructura dentaria.
- Agua: Hace que produzcamos más saliva, haciendo que la boca siempre este hidratada y de esta forma protegernos frente a bacterias y así evitar la formación de caries.
- Pescado azul: son una fuente de vitamina D, necesaria para la absorción de calcio y fósforo.
Esto son varios de los alimentos que no deberían de faltarnos en nuestro día a día, pero también cabe destacar otros alimentos que deberíamos de evitar para tener una buena salud dental.
Alimentos para evitar en tu dieta:
- Azúcares refinados
- Alimentos pegajosos, que harán que nuestra limpieza bucal sea más dificultosa.
- Refrescos y bebidas isotónicas
- Grandes cantidades de cítricos, aunque nos aporten vitamina C, en exceso nos hace que se deteriore el esmalte, produciendo su erosión.
- Hidratos de carbono, en exceso provoca inflamación gingival.
Tener una buena salud dental es importante para no derivar en problemas más serios. Para ello, ten una buena alimentación, cepíllate los dientes después de cada comida, emplea colutorios y usa seda dental.
